We are in-network with Blue Cross Blue Shield of Michigan (BCBS). The cost of an evaluation varies depending on your individual insurance plan, including your deductible, copay, coinsurance, and specific behavioral health benefits. Being in-network does not necessarily mean that an evaluation will be covered in full.

Neuropsychological and psychological evaluations may include several different services and CPT codes depending on the reason for the evaluation and the amount of testing required. Common codes used by our practice include:

• 90791 – Diagnostic/clinical intake
• 96136 – Psychological or neuropsychological testing, first 30 minutes
• 96137 – Psychological or neuropsychological testing, each additional 30 minutes
• 96132 – Neuropsychological evaluation services, first hour
• 96133 – Neuropsychological evaluation services, each additional hour
• 90846 – Family psychotherapy without the patient present, which may be used for certain parent-only feedback or consultation services when appropriate

We strongly recommend contacting BCBS before your appointment to verify your individual benefits. When calling the number on the back of your insurance card, you can provide the CPT codes above and ask:

1. Are these services covered under my plan when provided by an in-network psychologist?
2. Do I have a deductible, and how much of it has been met?
3. What is my copay or coinsurance for these services?
4. Is prior authorization or pre-certification required?
5. Are there any limitations or exclusions for psychological or neuropsychological testing?
6. Will I owe any portion of the testing cost after BCBS processes the claim?
